Those searches are won by businesses with clear pages for each experience and area, strong reviews and consistent details everywhere the engines look.

Plumbers, dentists, accountants and schools are compared within Claremont, Durbanville, Bellville or Somerset West, not across the whole metro, and the map results at the top of the page take most of the clicks. For those businesses, the Google Business Profile and a page per area matter more than anything else.
